28202c1d05 nvme: fix metadata handling in nvme-passthrough

On an NVMe namespace that does not support metadata, it is possible to
send an IO command with metadata through io-passthru. This allows issues
like [1] to trigger in the completion code path.
nvme_map_user_request() doesn't check if the namespace supports metadata
before sending it forward. It also allows admin commands with metadata to
be processed as it ignores metadata when bdev == NULL and may report
success.

Reject an IO command with metadata when the NVMe namespace doesn't
support it and reject an admin command if it has metadata.

e5d18854cb nvme initialize core quirks before calling nvme_init_subsystem

A device might have a core quirk for NVME_QUIRK_IGNORE_DEV_SUBNQN
(such as Samsung X5) but it would still give a:

    "missing or invalid SUBNQN field"

warning as core quirks are filled after calling nvme_init_subnqn.  Fill
ctrl->quirks from struct core_quirks before calling nvme_init_subsystem
to fix this.

Tested on a Samsung X5.

4722ac3b78 nvme: restrict management ioctls to admin
commit 23e085b2dead13b51fe86d27069895b740f749c0 upstream.

The passthrough commands already have this restriction, but the other
operations do not. Require the same capabilities for all users as all of
these operations, which include resets and rescans, can be disruptive.

366a2b3110 nvme: copy firmware_rev on each init

The firmware revision can change on after a reset so copy the most
recent info each time instead of just the first time, otherwise the
sysfs firmware_rev entry may contain stale data.

5f7fd71e5b nvme: Fix IOC_PR_CLEAR and IOC_PR_RELEASE ioctls for nvme devices

The IOC_PR_CLEAR and IOC_PR_RELEASE ioctls are
non-functional on NVMe devices because the nvme_pr_clear()
and nvme_pr_release() functions set the IEKEY field incorrectly.
The IEKEY field should be set only when the key is zero (i.e,
not specified).  The current code does it backwards.

Furthermore, the NVMe spec describes the persistent
reservation "clear" function as an option on the reservation
release command. The current implementation of nvme_pr_clear()
erroneously uses the reservation register command.

Fix these errors. Note that NVMe version 1.3 and later specify
that setting the IEKEY field will return an error of Invalid
Field in Command.  The fix will set IEKEY when the key is zero,
which is appropriate as these ioctls consider a zero key to
be "unspecified", and the intention of the spec change is
to require a valid key.

Tested on a version 1.4 PCI NVMe device in an Azure VM.

a25e460fbb nvme: fix a possible use-after-free in controller reset during load

Unlike .queue_rq, in .submit_async_event drivers may not check the ctrl
readiness for AER submission. This may lead to a use-after-free
condition that was observed with nvme-tcp.

The race condition may happen in the following scenario:
1. driver executes its reset_ctrl_work
2. -> nvme_stop_ctrl - flushes ctrl async_event_work
3. ctrl sends AEN which is received by the host, which in turn
   schedules AEN handling
4. teardown admin queue (which releases the queue socket)
5. AEN processed, submits another AER, calling the driver to submit
6. driver attempts to send the cmd
==> use-after-free

In order to fix that, add ctrl state check to validate the ctrl
is actually able to accept the AER submission.

This addresses the above race in controller resets because the driver
during teardown should:
1. change ctrl state to RESETTING
2. flush async_event_work (as well as other async work elements)

So after 1,2, any other AER command will find the
ctrl state to be RESETTING and bail out without submitting the AER.

We should be registering the ns_id attribute as default sysfs
attribute groups, otherwise we have a race condition between
the uevent and the attributes appearing in sysfs.

Resolve two context conflicts introduced by following two commits. These
two commits are applied after the current commit in upstream, while have
been merged into 4.19.y stable tree before the current commit.
1. drivers/nvme/host/core.c:nvme_ns_remove, introduced by commit
2181e455612a ("nvme: fix possible io failures when removing multipathed
ns").
2. drivers/nvme/host/multipath.c:nvme_mpath_set_live, introduced by
commit 5e416b11b4 ("nvme-multipath: fix possible I/O hang when paths

Commit 3b4b19721ec652 ("nvme: fix possible deadlock when I/O is
blocked") reverted multipath head disk revalidation due to deadlocks
caused by holding the bd_mutex during revalidate.

Updating the multipath disk blockdev size is still required though for
userspace to be able to observe any resizing while the device is
mounted. Directly update the bdev inode size to avoid unnecessarily
holding the bdev->bd_mutex.

Revert fab7772bfbcf ("nvme-multipath: revalidate nvme_ns_head gendisk
in nvme_validate_ns")

When adding a new namespace to the head disk (via nvme_mpath_set_live)
we will see partition scan which triggers I/O on the mpath device node.
This process will usually be triggered from the scan_work which holds
the scan_lock. If I/O blocks (if we got ana change currently have only
available paths but none are accessible) this can deadlock on the head
disk bd_mutex as both partition scan I/O takes it, and head disk revalidation
takes it to check for resize (also triggered from scan_work on a different
path). See trace [1].

The mpath disk revalidation was originally added to detect online disk
size change, but this is no longer needed since commit cb224c3af4df
("nvme: Convert to use set_capacity_revalidate_and_notify") which already
updates resize info without unnecessarily revalidating the disk (the
mpath disk doesn't even implement .revalidate_disk fop).

6f7baf41b7 nvme: Fix controller creation races with teardown flow

Calling nvme_sysfs_delete() when the controller is in the middle of
creation may cause several bugs. If the controller is in NEW state we
remove delete_controller file and don't delete the controller. The user
will not be able to use nvme disconnect command on that controller again,
although the controller may be active. Other bugs may happen if the
controller is in the middle of create_ctrl callback and
nvme_do_delete_ctrl() starts. For example, freeing I/O tagset at
nvme_do_delete_ctrl() before it was allocated at create_ctrl callback.

To fix all those races don't allow the user to delete the controller
before it was fully created.

The nvme multipath error handling defaults to controller reset if the
error is unknown. There are, however, no existing nvme status codes that
indicate a reset should be used, and resetting causes unnecessary
disruption to the rest of IO.

Change nvme's error handling to first check if failover should happen.
If not, let the normal error handling take over rather than reset the
controller.

011529b7d9 nvme: Treat discovery subsystems as unique subsystems

Current code matches subnqn and collapses all controllers to the
same subnqn to a single subsystem structure. This is good for
recognizing multiple controllers for the same subsystem. But with
the well-known discovery subnqn, the subsystems aren't truly the
same subsystem. As such, subsystem specific rules, such as no
overlap of controller id, do not apply. With today's behavior, the
check for overlap of controller id can fail, preventing the new
discovery controller from being created.

When searching for like subsystem nqn, exclude the discovery nqn
from matching. This will result in each discovery controller being
attached to a unique subsystem structure.

ffa56cbdd2 nvme: Discard workaround for non-conformant devices

Users observe IOMMU related errors when performing discard on nvme from
non-compliant nvme devices reading beyond the end of the DMA mapped
ranges to discard.

Two different variants of this behavior have been observed: SM22XX
controllers round up the read size to a multiple of 512 bytes, and Phison
E12 unconditionally reads the maximum discard size allowed by the spec
(256 segments or 4kB).

Make nvme_setup_discard unconditionally allocate the maximum DSM buffer
so the driver DMA maps a memory range that will always succeed.

5ac0da68ea nvme: provide fallback for discard alloc failure

When boxes are run near (or to) OOM, we have a problem with the discard
page allocation in nvme. If we fail allocating the special page, we
return busy, and it'll get retried. But since ordering is honored for
dispatch requests, we can keep retrying this same IO and failing. Behind
that IO could be requests that want to free memory, but they never get
the chance.

Allocate a fixed discard page per controller for a safe fallback, and use
that if the initial allocation fails.

When a PCIe NVMe device is not present, nvme_dev_remove_admin() calls
blk_cleanup_queue() on the admin queue, which frees the hctx for that

User space programs like udevd may try to read to partitions at the
same time the driver detects a namespace is unusable, and may deadlock
if revalidate_disk() is called while such a process is waiting to
enter the frozen queue. On detecting a dead namespace, move the disk
revalidate after unblocking dispatchers that may be holding bd_butex.

431f579a53 nvme: fix a possible deadlock when passthru commands sent to a multipath device

When the user issues a command with side effects, we will end up freezing
the namespace request queue when updating disk info (and the same for
the corresponding mpath disk node).

However, we are not freezing the mpath node request queue,
which means that mpath I/O can still come in and block on blk_queue_enter
(called from nvme_ns_head_make_request -> direct_make_request).

This is a deadlock, because blk_queue_enter will block until the inner
namespace request queue is unfroze, but that process is blocked because
the namespace revalidation is trying to update the mpath disk info
and freeze its request queue (which will never complete because
of the I/O that is blocked on blk_queue_enter).

Fix this by freezing all the subsystem nsheads request queues before
executing the passthru command. Given that these commands are infrequent
we should not worry about this temporary I/O freeze to keep things sane.

7436dc2ade nvme-multipath: revalidate nvme_ns_head gendisk in nvme_validate_ns

When CONFIG_NVME_MULTIPATH is set, only the hidden gendisk associated
with the per-controller ns is run through revalidate_disk when a
rescan is triggered, while the visible blockdev never gets its size
(bdev->bd_inode->i_size) updated to reflect any capacity changes that
may have occurred.

This prevents online resizing of nvme block devices and in extension of
any filesystems atop that will are unable to expand while mounted, as
userspace relies on the blockdev size for obtaining the disk capacity
(via BLKGETSIZE/64 ioctls).

Fix this by explicitly revalidating the actual namespace gendisk in
addition to the per-controller gendisk, when multipath is enabled.

f0c83dd15e nvme: fix possible io failures when removing multipathed ns

When a shared namespace is removed, we call blk_cleanup_queue()
when the device can still be accessed as the current path and this can
result in submission to a dying queue. Hence, direct_make_request()
called by our mpath device may fail (propagating the failure to userspace).
Instead, we want to failover this I/O to a different path if one exists.
Thus, before we cleanup the request queue, we make sure that the device is
cleared from the current path nor it can be selected again as such.

Fix this by:
- clear the ns from the head->list and synchronize rcu to make sure there is
  no concurrent path search that restores it as the current path
- clear the mpath current path in order to trigger a subsequent path search
  and sync srcu to wait for any ongoing request submissions
- safely continue to namespace removal and blk_cleanup_queue

If a controller supports the NS Change Notification, the namespace
scan_work is automatically triggered after attaching a new namespace.

Occasionally the namespace scan_work may append the new namespace to the
list before the admin command effects handling is completed. The effects
handling unfreezes namespaces, but if it unfreezes the newly attached
namespace, its request_queue freeze depth will be off and we'll hit the
warning in blk_mq_unfreeze_queue().

On the next namespace add, we will fail to freeze that queue due to the
previous bad accounting and deadlock waiting for frozen.

Fix that by preventing scan work from altering the namespace list while
command effects handling needs to pair freeze with unfreeze.

b2d5875685 nvme: validate controller state before rescheduling keep alive

Delete operations are seeing NULL pointer references in call_timer_fn.
Tracking these back, the timer appears to be the keep alive timer.

nvme_keep_alive_work() which is tied to the timer that is cancelled
by nvme_stop_keep_alive(), simply starts the keep alive io but doesn't
wait for it's completion. So nvme_stop_keep_alive() only stops a timer
when it's pending. When a keep alive is in flight, there is no timer
running and the nvme_stop_keep_alive() will have no affect on the keep
alive io. Thus, if the io completes successfully, the keep alive timer
will be rescheduled.   In the failure case, delete is called, the
controller state is changed, the nvme_stop_keep_alive() is called while
the io is outstanding, and the delete path continues on. The keep
alive happens to successfully complete before the delete paths mark it
as aborted as part of the queue termination, so the timer is restarted.
The delete paths then tear down the controller, and later on the timer
code fires and the timer entry is now corrupt.

Fix by validating the controller state before rescheduling the keep
alive. Testing with the fix has confirmed the condition above was hit.

5893e48f8f nvme: flush namespace scanning work just before removing namespaces

nvme_stop_ctrl can be called also for reset flow and there is no need to
flush the scan_work as namespaces are not being removed. This can cause
deadlock in rdma, fc and loop drivers since nvme_stop_ctrl barriers
before controller teardown (and specifically I/O cancellation of the
scan_work itself) takes place, but the scan_work will be blocked anyways
so there is no need to flush it.

Instead, move scan_work flush to nvme_remove_namespaces() where it really
needs to flush.

Whenever we update ns_head info, we need to make sure it is still
compatible with all underlying backing devices because although nvme
multipath doesn't have any explicit use of these limits, other devices
can still be stacked on top of it which may rely on the underlying limits.
Start with unlimited stacking limits, and every info update iterate over
siblings and adjust queue limits.
